Job Summary
Responsibilities:
Provide skilled nursing care including G-tube feedings and tracheostomy care
Monitor and document client health status and report changes as needed
Administer medications and treatments per physician orders
Collaborate with the healthcare and direct support teams to ensure safe person-centered care
Ensure compliance with DDI and state regulatory requirements
Knowledge & Abilities
- Strong clinical assessment skills and the ability to develop and implement care plans.
- Excellent communication and interpersonal skills.
- Proficiency in managing and administering medications.
- Ability to work independently and as part of an interdisciplinary team.
- Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
Qualifications :
Requirements:
Active LPN license in MD
Proven experience with G-tube and trach care
Previous experience working with individuals with developmental disabilities (DDI)
Strong documentation and communication skills
Reliable professional and compassionate
Additional Information :
Shift
Monday Friday 9am 5:00pm and flexibility to accommodate individual needs and schedule variations.
