What were looking for in a Case Manager - NN Healthy Families

To excel in the role of Case Manager for NN Healthy Families candidates should possess a Bachelors degree in human services (such as Social Work Psychology Sociology or Counseling) and at least one year of experience in case management services for children and adolescents. A valid Drivers License is essential for client visits and outreach activities. Strong communication skills both written and verbal are crucial for effectively collaborating with families service providers and other team members. The ability to conduct assessments develop treatment plans and provide counseling with empathy and professionalism is key to supporting clients in managing their physical social and personal needs.

Additionally organizational skills and the capacity to work independently while also being a team player in a supportive and customer-focused environment are vital for success in this role.

Knowledge and skills required for the position are:
