Job Summary

Key Responsibilities:

  • Supervise and coordinate the nursing care for residents during your assigned shift.
  • Assign duties to CNAs and monitor their performance.
  • Administer medications treatments and provide direct resident care in accordance with physician orders.
  • Participate in interdisciplinary care planning assessments and evaluations.
  • Communicate changes in residents conditions to the care team families and physicians.
  • Ensure compliance with care standards safety protocols and documentation requirements.
  • Participate in survey readiness training programs and policy implementation.
  • Act as a mentor to staff providing feedback and on-the-floor coaching when needed.

Qualifications:

  • Graduate of an accredited school of nursing.
  • Current Licensed Practical Nurse (LPN)license in the state of Pennsylvania.
  • Minimum 1 year of experience in long-term carepreferred.
  • Strong assessment critical thinking and communication skills.
  • Ability to lead others while maintaining a resident-centered approach.
