Key Responsibilities:
- Supervise and coordinate the nursing care for residents during your assigned shift.
- Assign duties to CNAs and monitor their performance.
- Administer medications treatments and provide direct resident care in accordance with physician orders.
- Participate in interdisciplinary care planning assessments and evaluations.
- Communicate changes in residents conditions to the care team families and physicians.
- Ensure compliance with care standards safety protocols and documentation requirements.
- Participate in survey readiness training programs and policy implementation.
- Act as a mentor to staff providing feedback and on-the-floor coaching when needed.
Qualifications:
- Graduate of an accredited school of nursing.
- Current Licensed Practical Nurse (LPN)license in the state of Pennsylvania.
- Minimum 1 year of experience in long-term carepreferred.
- Strong assessment critical thinking and communication skills.
- Ability to lead others while maintaining a resident-centered approach.
