Job Summary
In this role you will:
- Complete coordinate and transmit MDS assessments in accordance with ICF regulations and required timelines
- Ensure clinical documentation supports resident needs services provided and regulatory standards
- Partner with nursing leadership providers and interdisciplinary teams to maintain compliance and accuracy
- Track changes in resident status and update assessments as required
- Support person-centered care planning using accurate assessment data
- Prepare for surveys by maintaining organized compliant records
- Stay current on ICF state and federal regulatory requirements

Qualifications :
What we need from you:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Illinois
- Current MDS 3.0 certification is ideal but not required
- Minimum of 1 years of MDS experience in a Skilled Nursing Facility or ICF
