Job Summary

In this role you will:

  • Complete coordinate and transmit MDS assessments in accordance with ICF regulations and required timelines
  • Ensure clinical documentation supports resident needs services provided and regulatory standards
  • Partner with nursing leadership providers and interdisciplinary teams to maintain compliance and accuracy
  • Track changes in resident status and update assessments as required
  • Support person-centered care planning using accurate assessment data
  • Prepare for surveys by maintaining organized compliant records
  • Stay current on ICF state and federal regulatory requirements

Qualifications :

What we need from you:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Illinois
  • Current MDS 3.0 certification is ideal but not required
  • Minimum of 1 years of MDS experience in a Skilled Nursing Facility or ICF

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs) Short-term Care Rehabilitation Intermediate Care (ICFs) and Behavioral Health Service (SMHRFs) centers.
