Across our 60 locations we provide programs in a variety of service areas including workforce development children youth families seniors and homeless prevention.

BronxWorks has provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995. Our Stebbins Stabilization bed program provides transitional housing and case management services to a caseload of 64 chronically street homeless adults with the ultimate goal of ending their homelessness.

RESPONSIBILITIES

• Provide direct services to clients including intake engagement housing assessment.
• Manage a case load.
• Monitor and document client progress toward permanent housing goals.
• Document all interactions with or on behalf of clients.
• Maintain familiarity with program resources available on- and off-site.
• Conduct regular case conferences with clients and service providers as required.
• Complete program reports on a monthly basis or more frequently as required.
• Report to and meet with supervisor on a monthly basis.
• Conduct on-site home visits and other fieldwork as required.
• Perform additional duties as assigned.

PROGRAM SPECIFIC RESPONSIBILITIES

• Responsible for knowing all forms of housing subsidies.
• Responsible for making sure clients fill out housing applications correctly.
• Responsible for developing and maintaining housing resources.
• Responsible for keeping up with all new subsidized housing programs mandates.
• Responsible for completing monthly reports for agency and DHS.
• Participate in both in-house and off-site training to enhance their skills and effectiveness.
• Responsible for meeting with staff on a regular basis to discuss any problems.

QUALIFICATIONS

• Bachelors degree
• A valid Drivers license
• Proficiency in modern business communications including personal computers electronic mail voicemail facsimile and copier equipment.
• One years experience preferred.
• Excellent communication and interpersonal skills.
• Good writing skills.

PHYSICAL REQUIREMENTS

• Ability to use a computer for prolonged periods.
• Ability to occasionally lift and/or move up to 10 pounds.
• Ability to stand walk or sit for long periods of time.
• Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork.
• Ability to bend and retrieve objects and/or documents.
• Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
• Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work.
