Lead Registered Nurse (RN) Care Manager

Lead Registered Nurse (RN) Care Manager needed in Allentown PA.

Salary and Benefits

The salary range is based on experience ($74000 - $100000/year)

Responsibilities

Monday Friday (8:00 am 5:00 pm)
Ability to work one day a week remotely after training
Provide clinical care management support across OBGYN Pediatrics and Internal Medicine/Family Medicine
Conducts assessments of patient/family needs for the highest risk patients at risk for poor outcomes
Collaborate with the care team in assessing planning and implementing the patients care plan
Provide daily clinical and operational leadership for the Care Management team by serving as the primary clinical resource training and onboarding staff and coordinating cross-department collaboration

Requirements

Clear and active Registered Nurse license in the state of Pennsylvania
5 years experience of chronic management experience working in a care management capacity
1 year of experience or leadership experience preferred
Bachelor of Science in Nursing (BSN) required
