**Core Responsibilities:**
- Engage comprehensively in the Case Management Program (CMP) ensuring adherence to established standards for case management (CM) care.
- Leverage nursing expertise to navigate the CM process encompassing assessment planning implementation coordination and monitoring. Identify opportunities for CM enhancements and seamlessly integrate local CM methodologies.
- Formulate and execute localized strategies for case management using a mix of inpatient outpatient onsite and telephonic approaches.
- Develop and implement supportive tools for case management including those for patient identification and assessment clinical practice guidelines decision-making algorithms CM software and community resource databases.
- Synthesize CM with utilization management (UM) ensuring smooth integration between nursing case management and social work case management practices.
- Foster professional relationships with pertinent community organizations and agencies.
- Collect and accurately document patient care data for effective decision-making and continuity of care.
- Design comprehensive treatment plans that encompass preventive therapeutic rehabilitative psychosocial and clinical interventions ensuring a focus on optimal patient wellness.
- Establish robust mechanisms for the implementation of patient treatment plans and facilitate follow-up care in ambulatory and community healthcare settings post-discharge.
- Deliver targeted health care education to patients and/or caregivers tailored to identified learning needs.
**Work Schedule:**
- Monday to Friday requiring 8.5 to 9 hours of work between 7:30 AM and 5:30 PM inclusive of a lunch break.
- No weekend or holiday obligations with no on-call or callback responsibilities.
**Minimum Qualifications:**-
**Education:** Bachelor of Science in Nursing (BSN).-
**Experience:** A minimum of one year of experience as an RN Case Manager within the past three years.- **License:** Must possess an active unrestricted RN license.-
**Board Certification:** Required to obtain a case management-related certification (such as CCM CDMS CRRN COHN COHNJ-S ACCC CRC RN-NCM or CMC) within six months of hire. Employer reimbursement for certification costs is available.-
**Life Support Certifications:** Basic Life Support (BLS).-
**Security Clearance:** Must successfully pass a government background check and obtain the necessary government security clearance.
