We are seeking an experienced and strategic Transitions Manager to lead the planning and execution of large-scale operational transitions across our contracted health service environments. This role is ideal for a collaborative leader with expertise in project execution integration planning and change management who thrives in dynamic fast-paced environments.

Key Responsibilities

Lead the design and delivery of integration frameworks transition plans and operational readiness activities.
Ensure effective project governance compliance and milestone tracking to support successful service mobilisation and transition to business-as-usual.
Provide advice oversight and guidance to delivery teams throughout the transition life cycle.
Design communication and collaboration mechanisms that support engagement and clarity across stakeholder groups.
Oversee risk identification mitigation strategies and delivery of transition reporting to leadership.
Support continuous improvement by identifying and addressing systemic issues and recommending enhancements to operational processes.

About You

You bring strong commercial mindset proven change leadership and exceptional organisational capability. You will be confident engaging with diverse stakeholder groups and driving structured implementation outcomes.

Essential:

Bachelors degree in business commerce or equivalent experience.
Demonstrated experience successfully delivering change management or transition projects.
Strong leadership communication and stakeholder engagement skills.
Ability to plan prioritise and meet deadlines in complex operational environments.
Understanding of financial reporting and project control measures.

Desirable:

Experience in healthcare commercial project delivery or quality governance frameworks.
Experience working within contract-driven or operational program settings.
