Position Overview:

We are seeking a compassionate and dedicated Case Manager to support clinical operations for our Silver Spring office. This role is ideal for a nurse who is passionate about coordinating patient care supporting field staff and ensuring high-quality patient-centered services. As Case Manager you will play a vital role in fostering communication promoting patient well-being and ensuring compliance with regulatory standards and company policies.

Responsibilities:

• Oversee and coordinate patient care for the Silver Spring office.

• Provide support guidance and communication to clinical staff to ensure high-quality care delivery.

• Collaborate with interdisciplinary teams to create individualized care plans that promote patient health independence and dignity.

• Monitor and evaluate patient outcomes implementing follow-up or care adjustments as needed.

• Ensure compliance with all regulatory and accreditation standards.

• Support staff through ongoing education training and professional development.

• Build strong relationships with patients families and community partners.

Qualifications:

• Active RN license in Maryland.

• Minimum of 3 years of clinical experience in home health or a related healthcare setting.

• Experience in case management or care coordination preferred.

• Strong organizational communication and interpersonal skills.

• A passion for supporting staff and fostering a collaborative team culture.
