Job Description

Seeking an experienced Rehabilitation LPN to provide high-quality care to short-stay patients in a rehab environment with acuity levels slightly higher than traditional SNFs. The ideal candidate is dependable clinically strong and willing to work every other weekend as part of their regular schedule.

Responsibilities

Deliver direct nursing care to rehab patients in a short-stay setting.
Monitor patient conditions administer medications and perform treatments per care plans.
Support RN staff and collaborate with the interdisciplinary team to meet patient recovery goals.
Document assessments interventions and patient progress accurately in the EMR.
Assist with patient education and provide compassionate support to patients and families.
Follow all safety regulatory and infection-control guidelines.

Requirements

Active PA or Compact LPN License
BLS Certification
Minimum 1 year of LPN experience
