About the role
You will play a key part in ensuring participants receive high-quality person-centred support.
Office-based (approx. 15 hours per week):
Coordinate services rosters and day-to-day supports
Communicate with participants families and support workers
Maintain accurate documentation progress notes and updates to support plans
Help ensure supports align with NDIS goals and organisational standards
Field-based (approx. 15 hours per week):
Provide direct hands-on disability support
Assist with daily living community participation and skill-building
Deliver person-centred support that promotes independence dignity and choice
Build strong trusting relationships with participants and their support networks
What youll bring
Essential:
Certificate IV in Community Services Disability Ageing Individual Support or equivalent
Experience in disability aged care or community services
Strong communication and interpersonal skills
Good organisational and time management skills
Confidence using computers and completing documentation
Current drivers licence and reliable vehicle
NDIS Worker Screening Working With Children Check and Police Check required
Current First Aid & CPR Certificate
