Position Summary:
The Weekend Home Health RN provides patient-centered nursing care through the delivery and management of individualized care plans in the home setting. This role supports patients throughout Western Pennsylvania and surrounding areas and requires strong clinical judgment reliable documentation practices and the ability to independently manage time and travel. The nurse will provide weekend on-call coverage and work closely with the interdisciplinary care team to ensure high-quality care in the community.

Key Responsibilities:
Lead and collaborate in the management and delivery of patient care plans
Provide home health services throughout Western PA and surrounding areas
Spend approximately 20% of the day driving to and from patient locations
Consistently meet required charting and documentation deadlines
Self-manage schedule daily workflow and patient visit priorities

Schedule and Shift Details:
On Friday Saturday Sunday from 5:00 PM 10:00 PM
Scheduled to see 8 units per day on Saturday and 8 units per day on Sunday
Required to work 2 holidays per year that fall on a weekend
Must be able to complete orientation on two weekdays and on Saturday/Sunday

Travel:
Local travel required within the service area

Required Qualifications & Skills:
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum 1 year of experience in clinical documentation clinical informatics or clinical quality
Experience with EMR systems (including Practice Partner or other EMRs)
Valid certifications: ARC ALS and AHA ACLS
Ability to meet travel requirements and weekend schedule obligations
Strong time management communication and independent decision-making skills

Preferred Qualifications:
RN license number provided at submission
