Are you a passionate and dedicated Social Worker looking to make a real difference in the lives of adults with learning disabilities

Duties Include:

  • Working with adults who have complex social care needs whilst carrying a varied caseload with increasing complexity and autonomy.
  • Contribute to safeguarding assessments care planning and service delivery in collaboration with health and care partners.
  • Play a vital role in promoting personalisation ensuring services are shaped around the individual supporting independence and enabling adults to live fulfilling lives in a strengths based way.
  • Work collaboratively with individuals their families providers and other health care professionals to proactively encourage independent and safe living within the community.
