Job Summary

Job Title: Social Worker - Generic
Location: Darlington
Pay Rate: 26per hour

Liquid Personnel is recruiting anAdult Social Worker to join its clients Generic team based in Darlington.

What will your responsibilities be

Facilitate safe and timely hospital discharges using intermediate care or longterm support services ensuring a smooth home first transition.
Complete personcentred strengthsbased assessments and ongoing reviews to support recovery and determine longerterm care needs.
Work both independently and within the RIACT multidisciplinary team while maintaining highquality case recording and applying the Care Act 2014 and discharge procedures.

Qualifications and Experience:

To be successful in this role you must have;

Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
