Job Summary

Duties:

  • Performs Patient assessments: Accurately assesses patient s appropriateness for hospice care learning needs and progress toward goals.
  • Develops the plan of care for the patient and family Ensures appropriate utilization of services: Maintains visit productivity standards.
  • Demonstrates knowledge of insurance reimbursement Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers
  • Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary.
  • Documents patient education tools used Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
  • Revises care plan as indicated. Prioritizes care needs. Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
  • Shows evidence of interdisciplinary communication. Performs HHA supervisions per standards. Ensures appropriate utilization of services.
  • Follows scheduling process. Communicates progress toward goals with IDT.
  • Attends and participates in IDT meetings every 2 weeks. Evaluates the plan of care and revises as needed Maintains safe environment for patients and co-workers.
  • infection control measures and reports infections. Demonstrates knowledge of pharmacology and safe medication management. Routinely reconciles medications. Maintains current medication orders
  • Attends monthly staff meeting:Participates in PI meetings and activities/committees as requested. Accepts other assignments as requested.
  • Assumes responsibility for case management: Prioritizes schedule to accommodate patients needs. Documents case management activities. Demonstrates knowledge of pharmacology and safe medication management. Routinely reconciles medications. Maintain current medication orders.
  • Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities. Attends UR meetings as requested. Provides orientation/ mentorship to new employees.
  • Cross trains to learn new skills and demonstrates a willingness to teach other
  • Assists with growing agency business: Provides orientation to new employees.
  • Accepts other assignments as requested. Cross trains to learn new skills.

Skills:

  • Must have 2-3 years as RN in home hospice environment
  • Experience using an EMR excellent critical thinking and communication skills
  • Previous ICU exp. preferred

Education:

  • Current New Jersey RN License
  • BLS - required
