Job Summary
Location: Baker City
Position Overview:
We are seeking a dedicated and compassionate Hospice Nurse Case Manager to join our team in Baker this full-time role you will provide comprehensive nursing care and manage the overall care plan for patients receiving hospice services. Your expertise will ensure that patients and their families receive the best possible support during a challenging time.
Key Responsibilities:
- Conduct thorough assessments to develop personalized care plans for patients.
- Coordinate and manage patient visits ensuring alignment with their needs and physician directives.
- Manage medication administration and orders in a timely manner.
- Supervise and support LPNs LVNs and Hospice Aides as per established protocols.
- Collaborate with an interdisciplinary team to provide holistic care addressing physical emotional and spiritual needs.
- Educate families and caregivers providing resources to support them during this challenging time.
- Maintain a compassionate and respectful approach upholding the values of empathy and integrity in all interactions.
Qualifications:
- Associates degree preferred; Registered Nurse license required.
- At least one year of nursing experience with prior hospice experience favored.
- Willingness to participate in on-call rotations.
- Possession of reliable transportation and a valid drivers license.
