The Care Partner Lead provides leadership coordination and practice support to Care Partners delivering culturally safe independence-focused services to Elders and Aboriginal people living with disability. The role embeds wellness reablement cultural safety and self-determination across CHSP HACC-PYP and Support at Home while overseeing safety risk monitoring and appropriate escalation to the Clinical Nurse Lead. This is a non-clinical leadership role focused on coordinating mentoring and supporting the Care Partner team to work within scope and deliver high-quality care.

Position Purpose
The Care Partner Lead provides leadership workflow coordination and practice support to Care Partners delivering culturally safe independence-focused services to Elders and Aboriginal people living with disability.
This role ensures that wellness reablement cultural safety and self-determination are embedded across:
CHSP (Personal Care Social Support Domestic Assistance Respite)
HACC-PYP supports
Support at Home (non-clinical components)
Support at Home transition requirements
Everyday safety risk monitoring and concern identification
The Care Partner Lead ensures Care Partners work within scope follow support plans and escalate clinical concerns to the Clinical Nurse Lead. This is a non-clinical leadership role that coordinates mentors and supports the Care Partner team.
