Provide holistic case management assistance and support to clients within the scope of a Care Partner ensuring alignment with the Support at Home program objectives and best practices.
What youll be doing:
- Provide person-centred advice and support assisting clients in navigating the aged care system and making informed choices that align with their individual needs.
- Assist clients in identifying and reviewing personal goals and developing service plans within their available Support at Home budget.
- Coordinate and manage tailored support packages that enable clients to safely remain at home and actively participate in their communities.
- Ensure all client records and documentation are completed accurately and managed in line with organisational policies procedures and legislative requirements maintaining compliance and data integrity.
- Demonstrate awareness of and compliance with all relevant legislation including the Aged Care Act Aged Care Quality and Safety Commission Act Privacy Act and Freedom of Information laws.
- Actively contribute to the Continuous Quality Improvement Program ensuring best practices are embedded within Aged & Disability Services.
- Support the implementation of Occupational Health & Safety (OHS) initiatives promoting a safe environment for clients staff and service providers.
- Participate in the development implementation evaluation and review of policies and procedures to enhance service delivery within Aged & Disability Services.
The skills and experience youll bring to the role:
- Demonstrated knowledge and understanding of implementing and maintaining compliance with the Aged Care Quality Standards.
- Highly developed skills and experience in providing assessment and case management to high need and complex clients.
- Ability to liaise and consult with medical allied health and other community professionals at all levels.
- Ability to develop positive relationships with clients service providers and the community.
- Demonstrated computer proficiency and the use of client management software.
- Proven experience commitment and involvement in continuous quality improvement a safe working environment and person-centred care.
- Well-developed written and verbal communication skills including the ability to write reports prepare correspondence and establish rapport with the public and service professionals and resolve issues promptly and courteously.
- Knowledge of Occupational Health and Safety and risk factors associated with home care services.
- Proven experience in person-centred case management along with demonstrated knowledge and understanding of the principles of a CDC package
Required Certifications & Qualifications (If required)
- A Certificate IV Diploma or Tertiary qualifications in Allied Health Community Services Nursing or other related discipline coupled with relevant experience.
- Current drivers licence.
