Job Summary:
The Registered Nurse Case Manager is responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients providers and other members of our team.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Two years minimum of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Not specified
Special Considerations:
- May float between units based on unit needs
- Must have a vehicle for this assignment
- May be required to float to other facilities to meet needs
Scheduling:
- 24 Week Assignment
- Weekends required every other weekend
- Block schedules are unknown
- Ratios: 1:20
