Job Summary

$34.21 - $51.31

A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.

HSHS is seeking a RN Ambulatory Population Care Coordinator for the Physician Clinical Integration Network. Colleagues in this role will collaborate with at-risk population clients and their primary care providers to provide care that ensures the delivery of quality efficient and cost-effective healthcare services. Under general administrative direction they give patient education related to their illness and planned treatment provides medication management in collaboration with the primary care provider and promote patient self-management. Works in partnership with entire healthcare team to manage transitions of care and close gaps in care.

oDepartment: Accountable Care Services
oCore Function: Nursing
oSchedule: Full time 40 hrs./wk. 410-hour days or 58-hour days 7:30 AM to 5:30 PM or 8:00 AM to 4:30 PM.
oFacility: Springfield-Hollis Building
oLocation: Springfield IL

Education Qualifications
  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice is required.
  • Bachelors degree in Nursing is preferred.
Experience Qualifications
  • Home Health experience is preferred.
  • Case Management experience is preferred.
Certifications Licenses and Registrations
  • Licensed as a Registered Nurse (RN) in the state of practice is required.
