Minimum Qualifications

Four years of experience in providing health or developmentally disabled services counseling health care management or social services; OR

Six years of full-time work experience plus one year of experience in providing health or developmentally disabled services counseling health care management or social services; OR

A bachelors degree plus one year of experience in providing health or developmentally disabled services counseling health care management or social services; OR

An advanced degree in a social sciences field nursing public health; public hospital or health administration; speech-language pathology or communication sciences and disorders; physical occupational recreational or music therapy.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Job Duties and Other Information

Job Duties:

  • Develops and implements Plans of Support (POS) for individuals with Developmental Disabilities and their families within 30 days of case assignment and updates POS annually. This process involves scheduling appointments conducting face to face interviews assessing the need for services prioritizing needs making referrals to other available community resources making crisis requests for services planning and the coordination of services. This process involves being highly knowledgeable of available resources in the community. Follows procedures and implements program policy according to the operational guidelines.
  • Schedules and completes thorough interviews to determine the nature and onset of an applicants reported disability family history developmental history behavioral health conditions current natural supports current and future support needs and urgency of need.
  • Gathers information to make a determination of eligibility (gathers information from Early Steps applications school evaluations medical reports other service providers residential providers etc.)
  • Synthesizes information gathered from interviews and documentation to devise a written Support Profile which summarizes the applicants needs and choice of living options.
  • Collaborates with other service providers agencies school district staff social workers and medical and behavioral health professionals regularly for eligibility service and placement purposes.
  • Present information to the Entry Review Team on all denials of eligibility and questionable cases to assist the team in making a determination of eligibility
  • Complete an Individual Entry Review (IER) (Summarize data from all reports received to substantiate eligibility/ineligibility; determine onset of disability if person is eligible; determine if substantial deficits exist in adaptive functioning; and determine if the disability is a result of a mental illness) al
  • Develops and monitors Family Support contractual agreements which include individual/agency agreements and one-time purchases. This includes monitoring expenditures quarterly preparing agreements and invoices authorizing payment reviewing timesheets for accuracy and amending agreements as needed. Also documents and updates all address changes to the individuals file OCDD Participant Services Database electronic system and Louisiana Service Reporting System (LaSRS). Prepares and submits monthly and quarterly reports as requested to supervisor. Attends school transition ISC meetings or other transition/discharge meetings as needed. Manages demographic changes to OCDD Participant Services Database Louisiana Clinical Services and Louisiana Service Reporting System (LaSRS) as needed.
  • Completes Application Request with consumer family or case manager (requests are taken via telephone in person via fax e-mail etc.) This information is entered in the Participant Services Database andensuring the electronic and physical chart is updated with current documentation including a current SOA prior to forwarding to the family support unit.
  • Initiates the Urgent Triage & Diversion (UT&D) process for individuals who do not have a Personal Support Coordinator assigned in the CAHSD/DD office or Support Coordination Agency. Completes the Family Support Application gathers relevant information to present to management team for possible UT&D cases.
  • Independently acts as the designated ICF/IID certification representative for ICF/IID community homes. Incumbent independently determines if individuals with developmental disabilities meet admission criteria for ICF/IID facilities. Reviews ICF/IID policy and packets for accuracy as required by federal law. Completes a Notice of Medical Certification Form 142 and forwards to Medicaid Office and providers. Maintains files and social service narrative on all ICF/IID certifications which may include admissions transfers hospitalizations discharges death etc. Communicates with providers and Medicaid Office in regards certification. Follows procedures and implements program policy according to the operational guidelines.

Appointment Type: Probational or Job Appointment

Location:Capital Area Human Services Districts Developmental Disabilities Division.

Work Schedule: 8 hours per day Monday - Friday 10 hours per day Monday - Thursday or9 hours per day Monday - Thursday 4 hours on Friday.

Career Progression: This position may be filled as a Community Service Professional 1 2 or 3 depending on the level or experience of the selected applicant(s).

  • The maximum annual salary for the Community Service Professional 3 is $84302.00.
