YOUR NEW ROLE:
The Clinical Care Partner as a registered nurse or allied health clinician will utilise their knowledge skills background and clinical expertise to identify monitor and meet care recipient clinical care needs and reduce clinical risks.
As the clinical lead the Clinical Care Partner will provide clinical oversight of the Support at Home program and will include providing support advice and direction to the Care Partners regarding the clinical care needs and clinical care provided.
YOU WILL BE RESPONSIBLE FOR:
The Clinical Care Partner will provide clinical oversight of the SaH program which will include:
-Managing a caseload of care recipients with the most complex clinical care needs e.g. SaH
-Classifications 6-8 and those in receipt of the End-of-Life Care Pathway.
-Identifying clinical risks to care recipients health safety and wellbeing and implementing
strategies to manage those clinical risks.
-Leading ongoing care discussions and/or case conferencing with relevant health professionals
i.e GPs and other clinicians where required to ensure that clinical needs are identified and met.
-Regularly meeting with the Care Partners to provide support advice and direction in terms of
clinical care needs and clinical care provided.
-Conducting targeted care plan case note and incident/SIRs audits focussing on clinical care
identifying trends and developing and implementing strategies to reduce clinical risks including the
development and review of procedures.
-Representing SaH at the Divisional and/or Organisational Quality and Safety committee meetings which
includes managing and presenting procedural changes and conducting and presenting in-depth clinical case
reviews.
-Acting as the SaH staff representative at the Board Quality and Safety Sub-Committee.
-Provide support to the intake and leadership functions when high need/high risk care recipients are triaged
and clinical input/consultation is required.
-Conduct sign ups and undertake holistic assessment of needs and identification of clinical risks.
-Assist with managing assigned upgrades of existing Home Care Package or Support at Home levels based on
changing health needs and increased clinical risks.
WHAT YOU NEED TO SUCCEED:
Bachelor of Nursing/RN qualified or equivalent Occupational Therapist or Social Worker.
Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or other relevant professional association.
Experience with Case Management participant assessment and care planning in particular managing a caseload of participants from goal directed care principles and brokerage model of service delivery.
Sound Understanding of Support at Home and the Strengthened Aged Care Quality Standards.
Demonstrated ability to provide person centred holistic services.
Financial skills to effectively develop and monitor package expenditure for a variety of participants within a framework of participant directed care and choice supported by program guidelines and quality principles.
Highly developed written and oral communication skills interpersonal and organisational skills.
Demonstrated ability to work autonomously and as a member of an effective team including networking capacity with a wide range of service providers and other relevant stakeholders.
Demonstrated administrative and computer skills to meet the program requirements.
Knowledge of My Aged Care portal and Participant Information Management Systems.
Clinicians are expected to keep abreast of recent evidence-based therapies and/or treatments/practices/guidelines/frameworks related to practice/support/supervision.
Clinicians are expected to keep abreast of current research and identify relevant opportunities for innovation & research.
Clinician are expected to support and maintain all accreditation standards relevant to the discipline and services provided.
