ROLE: PROGRAM SCHEDULER FOR HOME HEALTH AGENCY

The Care Program Schedulers primary function is to facilitate scheduling of home visits for the care staff as requested by the client and assisting with the coordination of client care.
PRIMARY PURPOSE AND FUNCTIONS:
- Schedules home visits for the field staff as requested by the client and assists with the coordination of client care.
- Creates and maintains staff schedules for CNA-Caregivers and coordinates visits with Care Program Manager and other Team Members.
- Contacts Team Members and/or clients regarding day-to-day scheduling changes.
- Manages staffing for sick leave vacation long-term leave and other categories as may be needed.
- Manages emergency shifts to ensure adequate staff coverage.
- Resolves scheduling conflicts.
- Effectively manages multiple tasks by coordinating schedules prioritizing incoming requests and ensuring timely communication regarding client and staff schedule changes.
- Participates in the continuous quality improvement processes related to the client population.
- Provides effective communication to clients their families team members and other stakeholders.
- Notifies Administrator or Care Program Manager of any service delivery problems identified within the scheduling team.
- Ability to be on call as needed and resolve scheduling issues with little advance notice.
KNOWLEDGE AND SKILL SET:
- 2 years Home Health Scheduling Experience
- Experience in Home Health Intake and Admissions Scheduling or Orders Management a plus
- Ability to organize prioritize and multi-task
- Organized and resourceful self-starter strong ability to work independently and in a team.
- Excellent observation verbal written and interpersonal communication skills.
FOR QUALIFIED INDIVIDUALS OUR COMPENSATION AND BENEFITS PACKAGE INCLUDES:
- Starting base salary of $42500.00 per year.
