Job Summary

Care Coordination Nurse- LPN

GENERAL OVERVIEW

The incumbent is responsible for providing quality nursing assessments through the clinic to all independent living residents.

KEY DUTIES AND RESPONSIBILITIES

  1. Support the Mission Values and Vision of LifeSpire of Virginia.
  2. Responds to SARA pendant calls during shift.
  3. Provides appropriate nursing assessments and interventions as needed.
  4. Documents all encounters with residents/families in electronic medical record in a timely manner.
  5. Assures all medical record documentation is completed in a timely and accurate fashion after each visit.
  6. Provides follow-up with residents following all hospital admissions/discharges back to independent living.
  7. Provides follow-up with residents for all healthcare/off campus medical returns to independent living in a timely manner.
  8. Maintains daily log of movement of residents in and out of the community.
  9. Checks concierge reports every morning and follows up with residents as needed in a timely manner.
  10. Follows up on all resident health concerns and documents in the electronic medical record in a timely manner.
  11. Deliver direct nursing care as needed and when appropriate in accordance to state and federal laws/regulations and through application of acceptable nursing techniques standards objectives and goals.
  12. Assists in coordinating supportive services to include therapy pharmacy services and transportation.
  13. Provides back-up for the Lakewood First Clinic when needed.
  14. Provides in-home evaluation to residents as needed.
  15. Ensure compliance with regulatory agencies.
  16. Performs services outlined in the most recent Virginia Nurse Practice Act and within scope of license.
  17. Prepares laboratory specimens with proper lab slips and identification and arranges for transportation to the appropriate place as soon as possible after collection. Obtain lab results after specimens are submitted for processing; notification of physician regarding results followed by proper documentation.
  18. Works with residents for ongoing or routine monitoring including medication review. Keep these records up to date.
  19. Adheres to all professional standards set forth in Lakewoods Policies and Procedures.
  20. Establishes and maintains quality communications and a professional relationship with other care providers to include Nurse Practitioner Physicians Medical Director etc.
  21. Participates in on-the-job training and regular in-service training sessions; including necessary fire drills.
  22. Always strives for quality improvement to ensure quality excellence reduce risk liability and to enhance resident care and satisfaction.
  23. Maintains aware of and follows all written and verbally communicated expectations policies and practices.
  24. Perform other duties as assigned.

QUALIFICATIONS

    1. State of VA Licensed Registered Nurse or Licensed Practical Nurse.
    2. Must be able to attain CPR and AED certification within 30 days of hire.
    3. Ability to provide nursing services to residents which meets or exceeds Nursing Centers quality care and service standards.
    4. Ability to effectively supervise a diverse employee work group when applicable.
    5. Ability to effectively communicate with residents families supervisor and employees.
    6. Desire to work with the elderly.
    7. Ability to lift move and transfer residents while providing nursing care.
    8. Knowledge/experience in long-term gerontological nursing care.

WORKING HOURS AND ENVIRONMENT

Weekend and some holiday hours may be required. Some evening hours may be required as scheduled and business needs dictate. Work inside a well-lit well-ventilated environment. Must be able to tolerate occasional exposure to dust fumes odors water etc. as well as some noises. May be exposure to weather fluctuations in temperature (hot/cold) wet and/or humid conditions.
