Position overview
The Registered Nurse Acute Care / Long-Term Care provides licensed nursing care and clinical leadership for residents and patients across acute and long-term care settings in a community-focused medical center. This role supports person-centered dementia-sensitive care while ensuring regulatory compliance quality and safety across assigned households or neighborhoods.
Key responsibilities

Provide licensed nursing assessments ongoing evaluations and coordinated care for acute and long-term care residents.

Deliver clinical leadership across assigned households/neighborhoods supporting nursing staff and care teams in daily operations.

Oversee key clinical accountability areas including infection control safety wound care falls prevention and quality improvement initiatives.

Guide implementation of clinical policies procedures and best practices consistent with facility state and federal regulations.

Ensure documentation and care delivery meet regulatory standards for acute care and nursing facility services.

Foster collaboration with physicians therapists social services and resident/family representatives to support resident-centered care plans.

Promote person-centered care practices including dementia-sensitive approaches and honoring resident preferences and routines.
Required qualifications

Active unencumbered RN license (state-appropriate; must be current at time of submission).

Minimum 2 years of RN experience; background in acute care long-term care or skilled nursing strongly preferred.

Demonstrated leadership and clinical decision-making skills in resident- or patient-care environments.

Experience with regulatory compliance survey readiness and clinical documentation in acute and/or LTC settings.

Ability to support resident-centered and dementia-sensitive care practices.

Effective communication and team collaboration skills with interdisciplinary and family partners.

EMR experience preferred; EPIC or similar systems experience is an advantage.
