Role Scope:

The Growth teamfocuses on amplifying the impact of the PACE model of care byeducatingand expanding access tothe program. Our team manages the end-to-end process ofidentifying attracting engaging withassessingand enrolling new Growth Strategy &Operationswillsupportthedevelopment and implementation of strategic initiatives in support of the teams mission.

Core Responsibilities & Expectations for the Role

• Drivestrategic growth initiatives(comprehensively fromanalyses & research recommendations stakeholder alignment operational alignmenttorollout)across activities related to expanding the participant populationsuch as:

• New market strategy

• Top-of-funnel referral generation

• Operational improvements across the enrollment journey

• Callcenter strategy & operations

• Program design input

• Seamless handover to care teams

• Design of new operational processes

• Supportpartnermanagement(e.g. Kaiser Permanente our JV partner in California)including owning key relationshipsbuildingcollaborative tools and workflowsto educate/enable clinicians and refer patients who maybenefitfrom PACEandproblem solvingoperational challenges

• Support development and management of key Growth platforms and tools (including Salesforceand EPIC)

• Support regular reporting and performance trackingmonitorsuccess elicit feedback and input on initiatives andidentifyopportunities for / drive continuous iteration and improvement

• Collaborate across the organization including supporting key strategic decisions and building assets related to clinical operations participant experience and journeyhealth plan operationsand product development as they intersect with growth priorities

• Facilitate cross-functional & external stakeholder engagement to align on and drive progress against key initiatives

Qualifications:

• 5 years of relevant experience such as in health care delivery at a high-growth startupor consulting for health care payers & providers

• Demonstrated ability to be a self-starter with ability to excel in a fast-paced unstructured work environment comfortoperatinginambiguity and a bias to action

• Excellent analytical and project management skills

• Stronginterpersonal and written/oral communication skills; high EQ

• A bachelors degree isrequired.

• Strong problem solver who can connect high-level strategy to dailyexecution.

• Thrives in fast-paced evolving environments and builds structure where none exists

• Deep cultural alignment with Habitats values anda strong passion for our mission

• Willingness to work in a hybrid work environment with travel to Habitat Health centersas needed.

• Proof of medical clearance for communicable diseases including a TB test.

• Proof of all immunizationsareup to date.

• Proof of valid CA drivers license personal transportationgood drivingrecordand auto insurance as required by State law.

Preferred Qualifications:

• Advanced degree (e.g. MBA MHA MPH) in health care administration business or related field.

• Familiarity with the PACE program or other elderly care organizations.

• Experience working in geriatric primary care.

Compensation:

We take into account an individuals qualifications skillset and experience to determinefinalsalary. The expected base salary range for this position is $161000- $ actual offer will be at the companys sole discretion anddeterminedby relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
