Overview:
The Homelessness Addiction and Recovery Treatment (HART) Hub is a collaborative initiative designed to provide a seamless continuum of care through a culturally responsive and healing-centered model. Tailored to meet the needs of individuals facing homelessness and mental health challenges the Kenora HART Hub serves as a vital anchor for support within the Kenora geographical area and surrounding communities. By delivering coordinated compassionate and evidence-based services the HART Hub ensures equitable access to care. This approach integrates holistic and culturally grounded practices to promote stability healing and long-term recovery for members who are most vulnerable within our communities.
Reporting to the Chief Executive Officer the HART Hub Operations Coordinator is responsible for overseeing the HART Hub services operated by CMHA Kenora Branch. The HART Hub Operations Coordinator will work closely with the Clinical Service Hub Team at the Kenora Emergency Shelter and coordinate the co-located integrated care team that consists of both the internal HART Hub services operated by CMHA Kenora Branch as well as external partnerships that will be established through formalized Memorandums of Understanding. The HART Hub Operations Coordinator will ensure to coordinate the co-located onsite integrated care team services which will consist of low barrier access to a range of services through case management and transition support nursing peer support transportation support crisis stabilization support addiction services land-based cultural care connection and referral pathways to supportive housing support and onsite administrative addition the HART Hub Operations Coordinator will engage in system advocacy as well as education and consultation to the integrated care team both internal and external to the agency. The position will therefore oversee the service planning process of the HART Hub and evaluation reporting requirements.
Qualifications:
BSW MSW or registration with the College of Nurses of Ontario
Registration or eligible for registration with a regulatory College
Experience working with individuals who are homeless and marginalized who also have mental health and addiction struggles and/or demonstrated ability to work with this population
Personal qualities of empathy understanding and patience
Good interpersonal communication and relationship building skills
Ability to communicate professionally with community agencies
Knowledge of community resources
Cultural competence for diverse populations
Understanding and applied knowledge of Recovery Model
Understanding of trauma informed practice and implementation
Good documentation skills
Ability to learn and understand online scheduling
Proficient computer skills
Demonstrated understanding of personal space and ability to role model healthy boundaries
Ability to manage conflict and crisis situations
Leadership and supervisory experience
Demonstrated time management skills
Ability to work independently as well as in a team environment
Creative problem solving
First aid & CPR/AED Mental health first aid and NVCI or willing to obtain
Valid Drivers License
Duties:
- Adherence to all policies and procedures of Canadian Mental Health Association Kenora Branch
- Screening of HART Hub clients using approved clinical assessment tools to determine needs and triage to appropriate services (VI-SPDAT OCAN Assessment etc.)
- Ensure clients who are accessing the HART Hub are triaged to the Clinical Service Hub Team where appropriate (Clinical Service Hub Team Lead Service Navigator and Peer Support)
- Ensure clients are triaged to appropriate services that are co-located onsite by the integrated care team services which will consist of low barrier access to a range of services through case management and transition support nursing peer support transportation support crisis stabilization support addiction services land-based cultural care connection and referral pathways to supportive housing support
- Develop an integrated Recovery Plan in partnership with the identified integrated care team and strategies to support the achievement of goals and objectives of clients of the HART Hub
- Assertive engagement with clients of the HART Hub to connect them to services
- Make appropriate recommendations for support and services
- Consult and collaborate with the integrated care team regarding the support and integrated recovery planning for HART Hub clients
- Liaise with the integrated care team to assess client needs and the community
- Encourage understanding and support for clients by developing and maintaining working linkages with law enforcement primary care human service agencies and with informal community resources
- Continuous assessment of risk
- Continuous assessment of risk related to the clients housing needs substance use mental health needs social service needs and employment needs
- Advocacy or linking of clients with appropriate services supports and resources to meet the goals and objectives of the integrated recovery plan
- Conducting regular formal or informal evaluation of client satisfaction achievement of goals reassessment of service requirements and or needs in consultation with the client
- Direct provision of side-by-side support with clients where appropriate
- Provision of supportive counselling where appropriate
- Assist clients and provide modeling for problem solving and priority setting
- Referral to community agencies and services where appropriate
- Maintenance of client files consistent with policies and procedures of Canadian Mental Health Association Kenora Branch
- Outcome monitoring and evaluation as directed by the Chief Executive Officer
- Professionally representing Canadian Mental Health Association Kenora Branch
- Drafting all necessary correspondence and reports regarding clients and Team Lead reports
- Participation in administrative and office functions including ongoing maintenance of office space
- Participation in educational and professional development opportunities
- Ensure safety and security of all HART Hub clients during shift including interpersonal safety and security on the HART Hub services and integrated care team while onsite
- General administration duties
- Coordinate schedule of onsite services of the onsite coordinated team
- Prepare staff schedules that pertain to the HART Hub services operated by CMHA Kenora Branch and maintain scheduling program (approving time off requests etc.)
- Communicate changes and new rules/regulations to staff; ensure all operation and program policies and procedures are being followed
- Coordinate and facilitate orientation of the HART Hub staff of CMHA Kenora Branch
- Coordinate education in partnership with the Kenora Emergency Shelter Operations Team and Clinical Service Hub Team Lead of the co-located integrated care team on the safety protocols of the safety protocols of the Kenora Emergency Shelter
- Participate in recruitment of new staff
- Conduct staff supervision performance review and team meetings
- Attend education/training opportunities as required
- Continue to build relationships within the community through one-on-one networking group meetings and public outreach as directed by Chief Executive Officer
- Participate on specific committees or working groups as assigned by Chief Executive Officer
- Provide a safe environment by ensuring adherences to Workplace Health and Safety
- Actively participate in the development of sustainability of the ongoing Accreditation Standards Required Organizational Practices and Quality Improvement projects
- Regulations and Policies Infection Control Guidelines WHIMIS and Safe Work Procedures including but not limited to Fire Disaster and Evacuation Plans
- Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
- Ensure client safety and clients safety activities are emphasized to the co-located integrated care team
