Mid-levels in Practice: 4

% of Practice: 80% Onc 20% Hem (people w/ anemia)

EMR: Epic

Other Info:

- Administration of Chemo will be done by a Nurse Oncologist.

- Last appt. is typically at 430p; Break in the middle of the day.

- Sole provider practice; there will be resources from the other clinic.

- Locum will work in a medical group clinic but it is a hospital licensed space.

- Will not do any work in the hospital. The infusion space is within the clinic.

- Infusion space is only 4 chairs small service line; Putting a new radiation machine but not performing at the site.

Requirements:

Active CA License

BC/BE

CA DEA

BLS (required)

At least 2 yrs of current hospital clinical activity.

Gaps greater than 3 months must be explained
