_{Position Summary:}

_{The Physical Therapy Assistant (PTA) Home Health is responsible for providing in-home therapy treatments under the supervision of a licensed Physical Therapist. The PTA will carry out prescribed therapeutic exercises and modalities in accordance with the patients individualized care plan ensuring high-quality care in the comfort of the patients home. The role includes a variety of documentation and coordination responsibilities and serves patients across all age groups including infants pediatrics adolescents adults and geriatrics.}

_{Key Responsibilities:}

_{Administer physical therapy interventions as directed by the Physical Therapist.}
_{Provide treatment using appropriate modalities and therapeutic exercises.}
_{Accurately complete daily visit notes and discharge summaries.}
_{Maintain and manage therapy equipment and documentation in a home setting.}
_{Promote patient safety and independence in the home environment.}
_{Communicate effectively with patients families and the interdisciplinary care team.}

_{Oklahoma State Licensure as a Physical Therapy Assistant Required
(Or valid Oklahoma Temporary Permit under direct supervision pending licensure)}
_{BLS Certification Required}
