Job Summary

HCC Case Manager

MHMR is looking for a detailed oriented HCC Case Manager to join our team.

In this position of QMHP-CS you will be in a direct care position with the expectation that at least 50% of his/her work time is spent face to face with the individuals that MHMR of Tarrant County serves. This position has specializations that include functioning as a Rehabilitative Case Manager (RCM RCM-I) or a Clinical Coordinator (CC).A QMHP-CS functions as a part of the treatment team in the clinic to which he/she is assigned and provides assessment recovery planning and ongoing treatment in order to assist individuals with severe and persistent mental illness to gain the skills resources and supports necessary to attain their individualized recovery may be provided in the clinic or in the specific job function designation (RCM RCM-I CC) determines what percentage of time the QMHP spends in the clinic or the services in the community entails travel (often in the QMHPs own vehicle) and work throughout Tarrant County in a variety of neighborhood and home conditions.

Qualifications

Minimum Education Bachelors Degree

Defined Education: In Health or Human Services

Acceptable Degrees: Applied Behavior Analysis Child Development Counseling Early Childhood Education Early Childhood Intervention Education Psychology Gerontology Human Growth and Development Medicine Nursing Physician Assistant Psychology Registered Nurse Rehabilitation & Rehab Studies Social Work Sociology and Special Education

Substitutions

OR Bachelors degree with at least 30 hours of behavioral health coursework.

Required License(s)

Valid drivers license with an acceptable driving record.

Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.

Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
