JOB TITLE: Forensic Nurse Specialist- LPN
EMPLOYMENT TYPE: Part-time; 20 hours/week
SCHEDULED HOURS: Friday 8AM-12PM Saturday 5AM-1PM Sunday 5AM-1PM

ABOUT THE PROGRAM:DEPARTMENT OF CORRECTIONS
To be a member of the core team responsible for the provision of Medication Assisted Treatment to the
inmate population within Department of Correction facilities.
Responsible to:
Provide Medication Assisted Treatment for inmates with substance use or co-occurring disorders.
Function in collaboration with CHR OTP team Department of Corrections Safety and Security
staff and Department of Corrections Health and Addictions Services with the objective of
providing daily
DUTIES & RESPONSIBILITIES:
- Administer daily MAT (including methadone and buprenorphine) in accordance with physician/APRN orders and regulatory standards.
- Prepare and dispense individual doses ensuring accuracy and adherence to chain-of-custody procedures.
- Safely transport and secure controlled medications between CHR OTP clinics mobile clinics and correctional facilities in compliance with policy and regulatory requirements.
- Monitor patient response to treatment including conducting assessments and observing for side effects or adverse reactions.
- Conduct and monitor toxicology screens.
- Maintain accurate and timely documentation in the electronic health record (EHR) consistent with program and agency standards.
- Manage medication inventory including ordering restocking and reconciliation in compliance with CHR and regulatory requirements (DPH DMHAS DEA Joint Commission).
- Communicate effectively with prescribers clinical team members and DOC staff regarding patient care progress and safety issues.
- Participate in team meetings supervision trainings and other required activities.
- Ensure compliance with all DOC CHR and regulatory safety/security protocols.
- Perform other related duties as assigned.
- Per diem LPNs/RNs are required to provide coverage for at least one (1) weekend per month and two (2) holidays per calendar year.
- Additional duties as required.
QUALIFICATIONS:
Education: Degree/Certification in nursing field.
Experience: Experience with medication assisted treatment or correctional healthcare preferred.
Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut (depending on program/position requirements) required must have a valid drivers license. Must complete DOC background check and partnership agreement.
Training: CHR and DOC orientation.
