As a Case ManagerPsychogeriatric you will work independently or be part ofa team that co-ordinates a support network and links clientsliving with mental illnesses addictions and physical health
issues. The position attempts to empower the clientthereby increasing their independence dignity and self-respect in a safe and secure environment. Direct serviceand support will be provided to clients in areas such as dailyliving practical assistance crisis intervention linkages toprimary care and a range of other services to support theindividuals needs.
What You Will Do- Assists clients in the development and implementation of a care plan tailored to the clients needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues.
- Act as a liaison and client advocate among those involved with the clients care including family other caregivers medical and other multi-disciplinary providers.
- Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
- Provide supportive counselling or referral and linkages to health care programs or service providers.
- Provide crisis management/intervention and assist in the development of crisis plans for clients.
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
- Maintain strict confidentiality and follow all other required policies and procedures.
- Consistently interact with clients staff family members other community agencies and partners in a professional manner that reflects favourably on the organization.
- Attend and actively participate in all required training.
- Liaise collaborate and coordinate with otherprofessionals service providers and partners
- Complete other duties from time to time as assigned by the Coordinator and/or Program Director.
What You Bring
Must Haves- Completion ofa diploma/degree in Social Services/Human Services or equivalent;
- Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness addictions dementia or other neurological conditions;
- Group counselling related to mental health addictions and related issues.
- Knowledge of community resources and support
- An understanding of harm reduction recovery and psychosocial rehabilitation principles the therapeutic relationship and an ability to come to an agreement with client on goals/priorities.
- Crisis intervention skills with well-developed problem solving abilities and interpersonal skills
- Good oral and written communication skills and computer literacy including MS word and Excel.
- Counselling responding and assessment skills.
- Creativity and flexibility to approaches.
- Ability to work effectively as part of a team and interact with workers from a variety of agencies.
- Understanding of cultural competency and framework
- Possession ofa valid drivers licenseand a reliable vehicle in order to travel is required.
Nice-to- Haves- Completion of Gerontology is an asset.
- Ability to communicate in a different language an asset.
Work Environment:- Staff will be required to work in clients own homes and maytherefore be exposed to secondhand smoke.
- Occasionalevening and weekend hours may be required.
- Frequent travelwill be required using own vehicle.
