Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are critical.

Required Skills & Certifications:
- Active Registered Nurse license
- ACLS BCLS CPI NIH PALS certifications
- Minimum of 2 years of experience
- Clinical Assessment Medication Administration and Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Not specified
Special Considerations:
- May be required to float to another medical center
- The facility is able to call off once per two-week pay period
- RTO must be included at the time of submission
Scheduling:
- Variable start times:
- Every other weekend required (12 weekend shifts per contract)
- Schedule determined per manager discretion
