Job Title: Registered Nurse Case Manager

Overview: Opportunity for a Registered Nurse Case Manager to provide compassionate hospice care while supporting patients and families through coordinated clinical services. This role focuses on care management interdisciplinary collaboration and ensuring high-quality outcomes across hospice settings.
Responsibilities:

Manage and coordinate patient care in hospice settings

Develop and oversee individualized plans of care

Collaborate with physicians and interdisciplinary healthcare teams

Provide leadership guidance and clinical support to nursing staff

Ensure accurate and timely clinical documentation

Support patients and families with education and care coordination
Schedule:
-
Monday through Friday 8:00am to 5:00pm with varying start and end times
-
On-call rotation averaging approximately five shifts per month
Travel:

Local travel required across multiple counties
Compensation and Benefits:

Estimated annual salary of $80000

Qualifications:

Active Registered Nurse license in the state of Illinois

Minimum of one year of hospice experience

Experience in home health care home health management or Epic Home Health preferred

Management experience preferred

High school diploma or GED required
-
Current AHA BLS and ACLS certifications

Ability and willingness to travel locally as required
