Location: Santa Fe NM 87505
Duration: 32-week contract
Shift: 08:00 AM - 04:00 PM
Guaranteed Hours: 40 per week
Weekly Pay Rate: $1920 - $2120; (Negotiable based on experience)

Job Description:

We are seeking an experienced Registered Nurse Case Manager for a contract position at a healthcare facility in Santa Fe NM. The ideal candidate will have a strong background in discharge planning care coordination and interdisciplinary collaboration.

Key Responsibilities:

Coordinate patient care across the continuum focusing on safe and effective discharge planning
Collaborate with physicians nursing staff social workers and other healthcare professionals to develop individualized care plans
Evaluate patients medical psychosocial and financial needs to determine post-acute care requirements
Facilitate communication between patients families and care teams to ensure understanding of discharge instructions and follow-up care
Identify and address barriers to discharge including insurance issues home care needs and transportation
Document case management activities accurately and in compliance with regulatory standards
Monitor patient progress and adjust care plans as necessary
Ensure continuity of care by coordinating with external agencies skilled nursing facilities home health and other community resources

Required Qualifications:

License: Registered Nurse (RN) with a valid nursing license in the state of New Mexico.
Experience:
- At least 3 years of clinical nursing experience.
- Minimum 1 year of experience in case management.
- Must have experience in discharge planning.
Certifications (Required):
- BLS (Basic Life Support)
- Advanced Cardiovascular Life Support (ACLS)
