We are looking for a Complex Case Coordinator to join our Community Integration Service.
The Community Integration Service works with people who have complex social or other issues that create barriers for successful community living and supports them to problem-solve a pathway forward to independence.
About thisrole
The successful person will have experience in the mental health arena and have skills in collaborative planning for clients with complex needs thinking outside the square and developing service provision based onTe Whare Tapa Whaandstrengths models.
Whatyoullbe doing
- Working closely with clients through a transition period between hospital/residential setting and more independent living.
- Implementing a transition plan for all involved workerswhich may include Social Workers GPs Case Managers MSD family/whanauand friends. Clearlyidentifytasks for all includingtheclientandfollowingup on tasks.
- Record keeping through a PMS system which includesplans and ongoing actions as per Comcares procedural requirements.
Aboutyou
To be successful in this role you will have:
- Well-developedin theareaofcommunication and interpersonal skills.
- A high levelof literacy for planning and documentation.
- Problem solving/ solutions focused.
You must have a
New Zealand Certificate in Health and Wellbeing Level 4 - Mental Health and Addiction Support (or equivalent qualification).
Experience in the mental health and disability sector would be an advantage.
Applicants must hold a current full clean drivers license.
You will have a cultural awareness knowledge of and demonstrable commitment to the principles of TeTiritio Waitangi.
Eligibility
Applicants for this position must have the legal right to work in NZ and consent to undergo acriminal record checkprocess.
