Job Summary

3 Month Contract With A local Authority



Job Purpose

The Mental Health Hospital Discharge Senior Social Worker plays a key leadership role within the mental health discharge team. They are responsible for coordinating safe timely and person-centred discharges for individuals with severe and enduring mental health disorders. The post-holder will manage complex caseloads supervise and support other Social Workers and work collaboratively with multi-agency partners to ensure statutory duties are fulfilled and best practice upheld.



Key Responsibilities

  • Manage complex and high-risk cases involving individuals with severe and enduring mental health issues requiring discharge planning.

  • Conduct comprehensive social work assessments using a range of evidence-based frameworks and approaches.

  • Work autonomously exercising advanced professional judgment and decision-making.

  • Lead on safeguarding adults processes risk assessments and care planning.

  • Use knowledge of mental health legislation including the Mental Health Act Mental Capacity Act and Care Act to inform practice.

  • Provide professional supervision and support to a group of Social Workers (typically 36) including case audits reflective supervision and development planning.

  • Act as a mentor or "buddy" for newly qualified social workers and inductees.

  • Model high standards of practice and behaviour promoting a culture of learning and development.

  • Establish and maintain effective working relationships with health professionals hospital teams care providers housing and voluntary organisations.

  • Chair and lead multi-agency discharge planning meetings and professional meetings.

  • Prepare high-quality reports including those for Court of Protection safeguarding panels and internal governance.

  • Contribute to the development and delivery of team and service plans.

  • Participate in panel processes and team-based decision-making.

  • Produce and analyse performance data and management information to support service improvement.

  • Demonstrate cost-awareness and efficient use of resources in all aspects of service delivery.

  • Ensure adherence to legal and policy frameworks

  • Uphold the Professional Capabilities Framework at Experienced Social Worker level.

  • Remain up to date with current research evidence-based practice and legislative developments.

  • Identify and challenge discriminatory or oppressive practices appropriately.


Requirements

  • Qualified Social Worker with current registration with Social Work England.

  • Substantial post-qualification experience in adult social care including hospital discharge and mental health.

  • Experience managing complex cases safeguarding adults and working within multi-agency settings.

  • Previous experience of supervising or mentoring other social workers or social care staff.
