About the role

Tŷ Anwen is a residential mental health service in Caerphilly supporting up to 16 individuals living with serious and enduring mental illness and complex needs which may include self-harm.

Our team works to a recovery-focused model of care offering clear therapeutic boundaries and outcome-driven support that builds independence and confidence.

As a Complex Care Project Worker you will play a key role in empowering and supporting individuals on their recovery journey. Working as part of a dedicated team youll help to develop implement and review high-quality person-centred care and support plans that reflect each individuals needs preferences and aspirations.

What youll be doing:

  • Supporting individuals to live independently within a care setting.
  • Support prompting and administering medication.
  • Engagement in social activities
  • Completing and updating support plans risk assessments and risk management plans.
  • Carrying out supervisions
  • Completing assessments from referrals
  • Be responsible for the service in the absence of the manager.
  • Undertake relevant training for the role.
  • Liaise with Social Services Benefits Agencies Housing department and other relevant agencies
  • Organising group activities with the individuals at Ty Anwen or in the community
  • Providing emotional support to the individuals

Skills and Experience Needed

Please ensure your application evidences experience of the following:

  • I.T. literacy
  • GCSE Grade C or above in Maths & English (or equivalent)
  • Experience working as part of a team
  • Minimum of 2 years experience working with individuals with mental health needs

Pay and Pensions

  • 27297 (13.06 per hour)
  • Sleep-in rate: 62.64 per 8 hour shift - approx. 1 per week.
  • Waking night rate: 20% Enhancement from 11pm to 7am
  • Enhanced DBS which we will pay for
  • Social Care Wales registration is fully reimbursed

    • Defined Benefit (DB) 1/80th of final salary for every year of service.

Time off and Flexibility

  • 26 days holiday pro rata entitlement plus Bank holidays. Rising to 27 days after 1 years service or cash equivalent of 1 days annual leave.

  • Paternity Leave: 3 weeks
