Overview
We are seeking a compassionate and dedicated Registered Nurse (Home Health) to join our clients growing team in Libertyville this role you will provide high-quality patient-centered nursing care to individuals in their homes helping them achieve and maintain optimal health and independence. The ideal candidate will be clinically skilled self-motivated and passionate about delivering personalized care in a home setting.

Key Responsibilities
Perform comprehensive patient assessments and deliver skilled nursing care in home environments
Develop implement and adjust individualized care plans in coordination with physicians and the interdisciplinary care team
Educate patients and families on disease processes medications and post-care instructions
Monitor document and report changes in patient condition to ensure continuity of care
Ensure timely and accurate clinical documentation in compliance with agency and regulatory standards
Coordinate care with healthcare providers to promote positive patient outcomes

Schedule and Shift Details
Monday through Friday day shift
One weekend per month
Two to three on-call days per month
Two holidays per year
This is a Pay Per Visit position

Travel
Local travel required within the Libertyville area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community-based care
Strong clinical assessment organizational and communication skills
