Overview
The Start of Care Registered Nurse plays a key role in delivering high-quality compassionate care to patients in their homes. This position is responsible for performing admissions start-of-care assessments and initiating patient care plans in coordination with the healthcare team. The nurse ensures compliance with all care standards and documentation requirements while maintaining effective communication with patients families and other professionals involved in care.

Key Responsibilities
Complete start-of-care documentation for new admissions and post-hospitalization patients
Assess patients for telehealth eligibility and integrate telemonitoring into the plan of care when appropriate
Develop and implement patient care plans in collaboration with other healthcare professionals and paraprofessionals
Monitor patient conditions report changes to physicians and document all clinical findings accurately and promptly
Educate counsel and support patients and family members on nursing care needs preventive measures and rehabilitation procedures
Assign and supervise Home Health Aides with individualized care instructions
Participate in team meetings case conferences and performance improvement initiatives
Maintain ongoing professional development and fulfill continuing education requirements
Plan and organize daily schedules to optimize travel and maintain productivity
Adhere to all agency policies compliance regulations and patient care standards

Schedule and Shift Details
Monday through Friday 8 AM to 5 PM

Travel
Local travel required within the service area to meet patient care needs. Efforts will be made to maintain reasonable territory assignments.

Qualifications Required
Associate Degree in Nursing or higher
Active Registered Nurse (RN) license in Pennsylvania (PA)
Current certifications: Basic Life Support (AHA BLS) and Advanced Cardiac Life Support (AHA ACLS)
Valid drivers license and reliable transportation
Minimum of 2 years of nursing experience preferably in home health or start-of-care admissions

Preferred
Bachelor of Science in Nursing (BSN)
Familiarity with telehealth integration and home health documentation systems
Strong knowledge of compliance and home health regulatory standards
