Job Summary

Are you passionate about helping others find stability and thrive Join Heritage Health Network (HHN) a mission-driven organization caring for Medi-Cal members across six counties in California.

We are seeking a Housing Navigator to join our Enhanced Care Management (ECM) team. This role will help connect individuals experiencing housing insecurity to safe stable housing solutions while addressing their whole-person health needs.

What Youll Do:

Work with Medi-Cal members to secure housing and connect them with community resources.
Collaborate with HHNs multidisciplinary team (RNs social workers therapists and more) to deliver person-centered care.
Build trusting relationships and empower members toward independence and long-term stability.

Requirements

At least 2 years of relevant experience.
Location: Remote after the first month. In-person training is required in Riverside CA.
Schedule: Full-time MondayFriday
Bilingual Required: English/Spanish
