Logan Health is looking for a Registered Nurse to join their team as an RN Navigator for our new Dementia Program!

What youll be doing:

In this unique and impactful role youll work closely with our providers to help develop and shape Logan Healths new Dementia Program. As the RN Navigator you will deliver triage care navigation care management and direct in-home patient visits. You will be a vital source of daily guidance education and support for patients and families facing dementia-related conditions ensuring they dont have to navigate this journey alone.

Who you are:

You are an experienced RN with a strong background in caring for patients with dementia-related conditions. You work with confidence compassion and independence and you excel at providing clear education and guidance. You are comfortable driving to and from patient homes for visits and are able to manage your time effectively while working in the field. You are also able to support and educate Logan Health staff as part of this growing program.

Qualifications:

  • Current Montana RN License or a multi-state compact license with authorization to practice nursing within the state of Montana; required.

  • Minimum of one (1) of the following required:

    • Bachelors degree in a related field and four (4) years experience.

    • Minimum of six (6) years experience in a related area.

  • Minimum of four (4) years recent specialty experience in a minimum of one (1) of the identified specialty areas required:

    • Dementia

    • Clinical

  • Experience working with dementia patients is required.

  • Current Drivers License required.

  • Driving record for the previous thirty-six (36) months must not reflect any major driving offenses (i.e. DUI convicted of a felony involving a motor vehicle suspended license three (3) or more speeding tickets or other moving violations etc.).

  • In addition personal vehicles used for LH business:State law minimum liability coverage limits required.

  • Knowledge of professional practice standards and regulatory requirements as related to specialty care area required.

  • Knowledge and experience with electronic health records required.

  • Must function with a high degree of autonomy strong communication and interpersonal skills. Possess the ability to develop and implement community education outreach programs/workshops and seminars as defined for assigned specialty area required.

  • Possess ability to comprehensively assess patients psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s) required.

  • Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams required.

  • Promotes excellence through setting high standards and providing high quality outcomes.

  • Ability to act with integrity kindness and exhibit empathy.

  • Excellent organizational skills detail-oriented a self-starter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.

  • Works closely with members of the multidisciplinary care team and their recommendations. Exhibits professionalism and accuracy in interfacing with providers clinic staff patients and family members involved in the plan of care.

  • Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable. Assesses the physical psychological social and spiritual needs of the patient and family.

  • Identifies and assesses the learning needs of the population under care and/treatment. Builds therapeutic and trusting professional relationships with patients families and caregivers. Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).

  • Assists with the organization of the specialty interdisciplinary team meetings and facilitates the teams recommendations in a professional and timely manner.

  • Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).

  • Works collaboratively with all stake holders to identify patient needs and assists in care and services both medical and social throughout the continuum.

  • Collaborates with the navigation team and leadership to optimize processes and metrics to; reduce avoidable ED visits inpatient admissions hospital readmissions cost of care gaps in care increase patient satisfaction clinical outcomes and provider satisfaction as defined for assigned specialty care area(s)

  • Assesses education needs and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.

  • Facilitates the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.

  • Actively participates in activities that add value to organizational objectives and workflows such as process improvement projects and/or community-related initiatives to include community health fairs workshops etc.

Maintains regular and consistent attendance as scheduled by department leadership.

Shift:

Day Shift - Variable Hours (United States of America)

Schedule: Full-time 40 hours per week day shift

Logan Health operates 24 hours per day seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
