About the role
The purpose of the GP Shared Care (GPSC) Coordinator is to build maintain and promote strong partnerships with local GPs and the Central and Eastern Sydney Primary Health Network (CESPHN).
Through these partnerships and expert consultancy you will strengthen clinical pathways between the Community Mental Health Service (CMHS) and GPs.
Working directly with consumers who are preparing for discharge from the CMHS to their GP to facilitate a smooth transition into primary care.
Providing leadership and education to the mental health service to promote physical and metabolic health assessment and monitoring for consumers.
Facilitating access to primary care to address co-occurring physical health needs.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency (AHPRA) without condition.
At least 5 years full time equivalent post registration experience in mental health and/or holding post registration nursing/midwifery qualifications relevant to the field.
Demonstrated ability to work autonomously to deliver high quality clinical care to consumers with complex mental health needs.
Strong knowledge of contemporary issues and recommendations regarding metabolic and physical health needs of people living with mental illness.
Advanced communication and interpersonal skills with demonstrated ability to establish and maintain strong partnerships with a variety of stakeholders including GPs NGOs NDIS providers and other mental health services.
Experience in quality improvement and demonstrated commitment to continuing professional development.
Current unrestricted NSW drivers license.
