Pay Range:
$34.21 - $51.31
A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.
HSHS St. Johns Hospital is seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary care team. This role is essential to supporting patients throughout their hospital stay and transition of care by coordinating clinical services assessing discharge needs and ensuring optimal outcomes through timely and effective case planning.

Key Responsibilities:
- Conduct patient assessments to determine care coordination and discharge planning needs
- Collaborate with physicians nurses social workers and outside agencies to support patient goals
- Monitor patient progress and facilitate timely transition across levels of care
- Ensure compliance with hospital policies and regulatory guidelines
- Advocate for patient needs while promoting efficient use of healthcare resources
Education Qualifications
- Associates Degree or Diploma in Nursing is required.
- Bachelors Degree in Nursing is preferred.
Experience Qualifications
- 2 years of clinical experience is required.
- Prior case management experience is preferred.
- Quality/Process Improvement experience is preferred.
Certifications Licenses and Registrations
- License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
- Case Management certification (ACM CCM RN-BC) is preferred.
- Basic Life Support Certification (BLS) is required within 60 days of hire.
