Under the direction of the front desk lead the Behavioral Health Program Coordinator will assist with the integrated behavioral health and substance use disorder program at all United Community Health Center clinical locations. Within this role the qualified candidate acts as a liaison for patients and UCHC providers to assist in navigating behavioral health and substance use disorder treatment options and service availability within the UCHC communities. The goal of this position is to coordinate with clinic managers providers and front/back-office services to increase access to care and pursue more timely receipt of services. The coordinator will also help facilitate timely payment for services rendered by UCHC providers and gain a deep understanding of individual payer plans and coverage to better facilitate access to services. The coordinator will work closely with all UCHC departments to ensure the success of the program.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Serve as a patient advocate and coordinate on behalf of the patient.
2. Ensure implementation of UCHC policies and practices as developed for the program.
3. Gain a deep understanding of various insurance plans and coverage details for behavioral health services.
4. Identify key community organizations that provide services and assist in establishing better coordination of services for UCHC patients.
5. Assist with the implementation and coordination of Telehealth Psychiatric services.
6. Assist with Telehealth technology activation and coordination of Telehealth schedules for patient visits inside UCHC clinic locations.
7. Manage BH provider schedules to ensure department productivity.
8. Schedule patients coordinate visits and transportation scheduling and complete daily opening and closing of clinic.
9. Responsible for printing all necessary documentation for patient visit.
10. Performs intake triage vitals and assists patients with mental health screenings and document appropriately (PHQ Gad-7 Medication Summary PMP Vanderbilt Medical/Personal History form ADHD Questionnaire Autism forms Controlled Substance Agreement Form and/or any other necessary documentation).
11. Coordinate and perform Gene sight swabbing and documentation to process test.
12. Collect copays and past due payments from patients.
13. Coordinate all logistic support to BH department including supplies and equipment.
14. Abides and sustains workflow expectations established by supervisor.
15. Train new staff and assist in implementing new work flows as the needed.
16. Assist with all other program development and implementation needs regarding behavioral health and substance use disorders.
17. Help build positive relations within the team and all UCHC departments.
18. Schedule and organize meetings/events and maintain agenda.
19. Support growth and program development
20. Perform any other duties as deemed necessary by supervisor and or provider.
MINIMUM QUALIFICATIONS (KNOWLEDGE SKILLS AND ABILITIES)
Minimum of 3 Years of Behavior Health care coordination or working with behavioral health patients
Ability to work independently with strong sense of focus task oriented.
Ability to work with culturally diverse families and communities with the ability to be culturally sensitive appropriate and non-judgmental.
Detail-oriented and efficient
Excellent time-managementand organizational skills
Proficient in Microsoft Office computer literate
Previous behavioral health or medical experience preferred.
Work well under pressure and maintain a consistently pleasant and professional demeanor when communicating with others.
Is knowledgeable of and maintains HIPAA standards of privacy and confidentiality.
Strong interpersonal communication and written skills
Able to work in different work setting and environments.
Ability to solve practical problems and deal with a variety of concrete variables.
Follow through with patient inquires requests and complaints.
Adhere to UCHC policies procedures code of ethics and mission.
LICENSURE/CERTIFICATION
Current CPR certification
Valid Arizona drivers license

Experience working in health industry

TRAVEL REQUIREMENTS
Ability to travel to satellite sites and/or off-site locations for events or training as needed.
