The Opportunity

We are seeking a hybrid Social Worker/Discharge Planner to work with our team at Wollongong Private Hospital. As a member of the Allied Health team you will be working in a key role that provides clinical leadership to nursing medical and allied health providers. You will be responsible for providing an effective discharge planning service coordinating the strategic and operational planning for discharge activities and associated services across the hospital. You will deliver personal counselling helping individuals and carers adjust to their current medico-psycho-social situation.

Your role will involve but is not limited to;

  • Work providing social work services including counselling and community follow ups
  • Coordinating the patient discharge planning process
  • Ensuring patients have a smooth transition from the hospital to the community with appropriate care and services
  • Planning complex discharges with the multidisciplinary team.

Skills and Experience

To succeed in this role you will bring a strong clinical skillset with experience working in an acute care hospital setting. Your effective interpersonal and communication skills (both written and verbal) ability to work in a multidisciplinary fast-paced team environment client-focused attitude and willingness to accept and promote Ramsays core values are essential for this role.

You will also demonstrate:

  • Degree in Social Work or equivalent
  • Eligibility for membership of the Australian Association of Social Workers or equivalent Professional Association
  • Minimum of 2 years relevant clinical experience in an acute health care setting
  • Extensive knowledge of community services and other resources relevant to this role.
  • Knowledge of and ability to implement contemporary discharge planning practices
  • Demonstrated ability to effectively and efficiently manage a caseload.
  • Excellent interpersonal and communication skills.
  • High standard of customer service and a team approach.
  • Commitment to professional development and maintenance of current evidence-based practice.
  • Flexible and positive approach to work.
  • Experience with My Aged Care the residential aged care process and the NDIS will be highly regarded
