What Youll Do:
As an LPN youll play a vital role in supporting our patients and RN Case Managers by:
Following the individualized plan of care and physicians orders to deliver safe high-quality nursing care.
Teaching patients and families simple self-care techniques to support independence.
Monitoring patient progress reporting changes in condition and suggesting updates to care plans.
Providing positive compassionate communication to patients families and caregivers.
Helping prevent unnecessary ER visits or hospitalizations by delivering proactive care at home.
What You Need:
- High School Diploma or GED
- Graduate of an accredited LPN/LVN program
Current State License (LPN/LVN)
Minimum 1 year of clinical experience (home health or hospice a plus)
Reliable transportation valid drivers license and insurance
Ability to sit stand bend lift and move intermittently with the capability to lift to 50 - 100 lbs. as needed.
The base salary for this position is $25.00 to $37.00 per hour based on the companys good faith estimate at the time of posting. Actual pay will be determined based on factors such as education experience skills and internal equity.
This role may also be eligible for additional compensation which could include performance-based bonuses commissions on-call pay or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
