As part of the Supportive Housing team the Case Coordinator Street2Home plays a vital role in supporting people experiencing homelessness and rough sleeping to access appropriate services build capacity and work towards safe sustainable housing and greater community participation. The position strengthens participants ability to coordinate and implement supports develop resilience and maintain tenancies while also preparing those sleeping rough for independent living.
Working from both a home-visit and assertive outreach capacity the Case Coordinator assists participants to navigate complex service systems by linking them with appropriate providers sourcing and coordinating a range of funded mainstream and informal supports and responding to points of crisis. The role includes responsibility for case planning and coordination across multiple domains which may include supporting applications to the NDIS facilitating compliance with corrections or court orders and assisting with referrals such as Victims of Crime.
Through a participant-focused strengths-based approach the Case Coordinator contributes to the overall aims of the Street2Home program by enhancing housing stability social inclusion and long-term wellbeing.

You will hold a relevant qualification in Community Services Social Work or a related field (Diploma level or above) and bring a strong understanding of person-centred and human rights-based approaches. You are committed to working alongside people experiencing homelessness and rough sleeping supporting them to achieve their goals strengthen their independence and build meaningful connections in the community.
You have demonstrated experience in case coordination and can confidently support clients with complex needs including navigating multiple service systems and responding to social and environmental challenges. You are skilled in developing and implementing individual support plans coordinating referrals and working collaboratively with a range of providers to achieve positive outcomes. You are also confident in reviewing client needs and allocating referrals across both the Assertive Outreach and Supportive Housing Teams ensuring participants are matched to the right supports.
Your strong communication and problem-solving skills enable you to effectively engage with clients colleagues and external stakeholders including in times of crisis. You can provide guidance in resolving complex cases and supporting crisis presentations working alongside others to deliver timely and effective responses. You are able to foster resilience promote inclusion and model a flexible creative approach to supporting people in diverse circumstances. You also bring a detailed knowledge of local networks and services and are proactive in building partnerships and strengthening community capacity.
Organised and adaptable you are confident with reporting documentation and the use of technology to support quality service delivery. You can contribute to program evaluation reporting and continuous improvement initiatives and you are comfortable assisting in guiding or mentoring team members when required.
