What youll do:

Deliver nursing procedures and clinical assessments
Develop and coordinate care plans tailored to clients needs
Manage and monitor ongoing care in collaboration with families & the care team
Build strong supportive relationships with clients in their homes

What youll bring:

Current AHPRA RN registration experience in community-based nursing
Strong interpersonal problem-solving & communication skills
Ability to work well within a multi-disciplinary team
Current drivers licence registered vehicle
Valid working rights in Australia
National Police Check NDIS Worker Check (or willingness to obtain)
