Must have min of 1 years exp as a RN Case Manager or 3 years as an PALS are NRP TNCC CNOR are preferred for each appropriate duty section at the time of employment and throughout as State Lic!Contractor will work with outside agency case managers and/or social workers of hospitalized (primarily complex medical geriatric surgical pediatric obstetric gynecologic and trauma patients) who utilize location as their primary care network. Contractor to help with case management consult requests for all ambulatory Development: Degree or diploma from a professional registered nursing program approved bythe legally designated State accrediting agency at the time the program was completed by the with the care coordination of ordering complex durable medical equipment and care managing mostly un-resourced patients/referrals but may include some resourced patients/referrals. Work with outside agency case managers and/or social workers of hospitalized (primarily complex medical geriatric surgical pediatric obstetric gynecologic and trauma patients) who utilize location as their primary care Schedule:5.2.1 Work Shift: 8 Hours - (8:00 a.m. 4:30 p.m.) with 30- minute non-paid lunch. The contractor must be assignedhours bi-weekly; scheduled for weekends and Federal holidays; no on call; as scheduled by the COR in writingbased on the facilities need.5.2.2 Scheduling of Shifts: The COR or Alternate COR at the IHS facility will determine specific tour of duty. Thecontractors employee must work 100% of the contracted hours.5.2.3 Work Flexibility: As directed by the COR the Contractors employee must rotate into other duty sections asneeded to support patient care.5.2.4 Work Absence: The Contractor will perform and obtain approval from their agency who will inform the CORand/or Alternate COR prior to any absence from work. If the length of the absence exceeds working hours the Contractor must request from agency approval at least fifteen (15) days (the exception is verifiable emergencies) in advance of the desired absence.
